Use this form ONLY if one or more of your J-2 dependents are departing permanently from the U.S. with NO expectation to return in J-2 status during the remainder of the J-1 principal's program duration.

If you have more than three dependents to report, please fill in an additional form. This form is only to be used by J-1 Exchange Visitors on UC San Diego's Exchange Visitor program.
